Psychotic symptoms can begin abruptly in someone with little or no previous history. A brief episode may fully resolve, but the eventual diagnosis often depends on what happens over time.
Schizophrenia is a long-term condition that can affect perception, thought organization, motivation, emotion, and functioning. Psychosis is one part of the condition, not the person’s identity.
Delusional disorder usually involves one or more fixed beliefs while speech, cognition, and everyday functioning remain comparatively organized outside the belief’s impact.
Schizoaffective disorder combines substantial mood episodes with psychotic symptoms that also have some independence from the mood state. Diagnosis requires a longitudinal history rather than one interview.
During severe mania or depression, bipolar I disorder can include delusions or hallucinations. In bipolar psychosis, the psychotic symptoms track the mood episode.
Psychotic depression is a severe depressive episode accompanied by delusions or hallucinations. The psychosis occurs within the depressive episode and often reflects guilt, ruin, punishment, disease, or death.
Trauma can produce hypervigilance, mistrust, flashbacks, dissociation, sleep disruption, and a persistent sense of threat. Some people also have separate psychotic symptoms, but the boundaries require careful assessment.
Intoxication, withdrawal, prescribed medicines, supplements, or drug interactions can cause psychosis. The timing between exposure and symptoms is a central diagnostic clue.
Brain, immune, endocrine, infectious, metabolic, seizure, and other medical conditions can directly cause psychosis. New-onset or atypical symptoms may require urgent investigation before a primary psychiatric explanation is assumed.
Delirium is an acute, fluctuating disturbance of attention and awareness caused by a medical problem, substance, medication, or withdrawal. Neurocognitive disorders produce a more persistent decline in memory or other cognitive abilities.
In a close, isolated relationship or group, one person’s fixed beliefs can become adopted or reinforced by another person. Contemporary classification focuses on each person’s symptoms while preserving the relational context in the clinical formulation.
Long-standing mistrust, unusual beliefs, ideas of reference, or brief stress-related paranoia can occur in personality pathology without the persistent loss of reality testing seen in a primary psychotic disorder.
AI-associated psychosis is not an established standalone diagnosis. Emerging case reports and analyses describe situations in which immersive chatbot use appears to reinforce, elaborate, or become incorporated into delusional or manic experiences.
